Pathways to Planning.
Parent to Parent provides planning that focuses on the gifts, capacities and strengths of people with a disability.
Families support other families and people with a disability to plan a desirable future by assisting them to identify the steps necessary to move toward that future. This is a valuable and practical way to look at a vision that sees people with a disability as belonging in their community. Planning assists families to develop practical skills, to assist them to negotiate paid and unpaid support.

Essential Lifestyle Plans
A Lifestyle plan provides information on how people can best be supported. They focus on balancing what it important to and important for the person, whilst keeping their skills and gifts as a focal point.

Planning Alternate Tomorrows with Hope
P.A.T.H. is a person centred planning tool that provides a graphic plan for the future. It records where people are now and their journey towards a desirable future.

These are valuable and practical methods of looking at the future, writing down ways of taking action that promote people with a disability to actively participate in their community.
Families have said that by having Family Facilitators assisting them in the process they feel supported. They feel secure in sharing their dreams and have confidence in the skills of the facilitators.
